Application form for the establishment of state allowance to families raising children
List of documents to provide at the place of work for the establishment of allowance on the birth of a child
- applications*;
- passport or other identity document*;
- birth certificate if a child was born in the Republic of Belarus*;
- birth certificate if a child was born outside the Republic of Belarus*;
- birth and death certificates of children, including those over the age of 18 (submitted for all children)*;
- court decision copies on adoption (hereinafter – adoption) for families who have adopted (hereinafter – adopted) children;
- extracts (copies) from the employment records of parents (adoptive parents) (hereinafter-adoptive parents, guardians) or other documents to confirm their employment if it is necessary to determine the purpose of the allowance;
- court decision copies on marriage dissolution or tmarriage dissolution certificate or other document to confirm the category of a single-parent family (for single-parent families);
- decision copies of the local executive and administrative body on the establishment of guardianship (guardianship) for persons appointed as guardians of a child;
- marriage certificates if the applicant is married*.
List of documents to provide at the place of work for the establishment of allowance to women registered with state health organizations before the 12-week gestation period:
- applications;
- passport or other identity document;
- conclusions of the medical consultation commission*;
- extracts (copies) from the employment records of the applicant and the applicant’s spouse or other documents to confirm their employment if it is necessary to determine the purpose of the allowance;
- court decision copies on marriage dissolution or marriage dissolution certificate or other document to confirm the category of single-parent family (for single-parent families);
- marriage certificates if the applicant is married.
List of documents to provide at the place of work for the establishment of child care allowance for children under 3 years of age
- applications;
- passport or other identity document;
- birth certificates (if two or more children are brought up in a family – at least two birth certificates) (for foreign citizens and stateless persons who have been granted refugee status in the Republic of Belarus if such certificates are available);
- court decision copies on adoption (for families who have adopted children);
- decision copies of the local executive and administrative body on the establishment of guardianship (guardianship) for persons appointed as guardians of a child;
- disabled person certificate or conclusions of a medical rehabilitation expert commission (for a disabled child under 3 years of age);
- the Chernobyl disaster victim certificate (or other radiation accidents) for citizens who permanently live in the territory that has been exposed to radioactive contamination in the zone of subsequent resettlement or in the zone with the right to move out;
- marriage certificates if the applicant is married;
- court decision copies on marriage dissolution certificate or other document to confirm the category of single-parent family (for single-parent families);
- information about the period for the maternity allowance was paid;
- extracts (copies) from the employment records of parents (adoptive parents, guardians) or other documents to confirm their employment if it is necessary to determine the purpose of the allowance;
- certificates to confirm the citizen is a student;
- certificates to leave for work or service before the expiration of parental leave for children under 3 years and termination an allowance payment when applying for parental leave before a child reaches the age of 3 by another family member or relative of a child;
- family composition certificate and registration of a child in the housing department;
- information about an allowance amount for a child and the payment period in case of a change of an allowance payment place;
- extract from the order on registration of parental leave for children under 3 years (HR department);
- extract from the order on the duration of the employment contract (contract) (HR department);
- certificate on you are not registered as an individual entrepreneur.
